Ice Show Form:

If you intend to participate in the ice show this year, we need to have the form filled out and returned on the dates indicated. The ice show fee and the payment for banquet tickets are due with the form. Measurements for costumes will be taken when the forms are submitted. If you are able to volunteer some time to help out with the ice show organization and preparation, please indicate that on the form. The ice show is for ALL skaters in our programs, including Pre-CanSkate and all levels of CanSkate. It is an enjoyable event for all skaters. Thank you.
